What you will do:
The Home Care RN Case Manager is a professional nurse who coordinates and directs the home care patient's care in their home environment (private residence, assisted living, etc.) based on individual needs. The Home Care RN Case Manager is responsible for independent management of the Home Care patient population requiring use of advanced assessment, teaching, and decision making skills. The Home Care RN Case Manager cares for a caseload of home care patients by evaluating the patient for appropriateness of home care needs and developing the plan of care in collaboration with the provider, caregivers, and Home Care IDG members. The Home Care RN Case Manager provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Home Care RN Case Manager provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines, and best practice. The Home Care RN Case Manager documents in the patient's medical record in a timely manner according to agency policy. The Home Care RN Case Manager attends and actively participates in Case Conference and team meetings. The Home Care RN Case Manager performs on-call responsibilities.
Required Education:
Graduate of an accredited Diploma of Nursing Program or Associate's Degree in Nursing.
Preferred Education:
Bachelor's Degree in Nursing
Required Skills:
Nursing skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Preferred Skills:
Experience with Home Care Home Base; Experience with smart phones and smart devices
Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver's license; Current auto insurance. Must have reliable transportation.
Preferred Licensure/Registration/Certification:
OASIS certified; Wound Care certified; Advanced education or certification in area of specialty
Required Experience:
One year experience in a healthcare setting in respective discipline
Preferred Experience:
Medical-Surgical experience; Experience in Case Management; Supervisory position in healthcare setting; Home Care of Hospice experience
