Job Description: Care Partner (RN, LPN, or LVN)
Location: Remote (NY & NJ-based patients)
We're looking for a compassionate, detail-oriented clinician to join our team as a Care Partner. In this role, you'll serve as a key touchpoint for patients recently discharged from New York hospitals after a stroke or brain injury. Using the PRISM platform, you'll hold virtual check-ins, monitor health data, and coordinate care to ensure patients stay on track in their recovery.
This role is ideal for someone who thrives on patient connection, loves solving problems, and is excited about the possibilities of remote-first healthcare. Over time, this position may transition to fully remote depending on your preferences.
What You'll Do:
- Meet virtually with patients to review blood pressure readings and symptom trackers, answer questions, and provide post-stroke guidance
- Conduct remote motor assessments to track recovery progress and spot any red flags
- Analyze trends in patient data and take action through established escalation protocols
- Coordinate with hospital staff, rehabilitation teams, and medical partners to ensure seamless care
- Handle light administrative duties like care documentation and patient onboarding
What We're Looking For:
- RN, LPN, or LVN with an active NY license (multi-state license is a plus)
- 3+ years of clinical experience
- Experience working with stroke survivors or patients with neurological conditions preferred
- Prior exposure to remote care, virtual care platforms, or early-stage health tech companies is a plus
- Bilingual (especially Spanish-speaking) candidates preferred
- Tech-savvy, organized, and comfortable working independently in a fast-paced environment
- Strong clinical judgment and communication skills
