What to Expect: The Community Health Worker (CHW) will be responsible for developing community partnerships, leading outreach efforts, and helping individuals navigate and access community services, resources, and adopt healthy behaviors. The CHW will primarily be working out in the community with specific target populations. The CHW will work closely with medical providers, healthcare teams, and other community agencies to improve access to services, improve community health outcomes and decrease health disparities. The CHW will assist in providing effective, equitable, understandable, and respectful quality care and services that are responsive to diverse cultural health beliefs and practices, preferred languages, health literacy, and other communication needs. The CHW will advocate for individuals and community health needs and assist in providing services, including home visits and health screenings.
Essential Duties
Data Analytics and Assessment Skills
1. Contribute to data collection efforts to ensure that all programmatic forms and documentation is sufficient for tracking program outcomes.
2. Participate in Sheboygan County Division of Public Health's community health assessment and health improvement planning and intervention activities including qualitative and quantitative data collection efforts.
Policy Development and Program Planning
1. Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
2. Ability to establish priorities for service intervention and support for families seeking services.
3. Schedule, facilitate, and provide educational opportunities for community members and partners regarding nutrition/food safety, disaster preparedness, health and/or medication, and financial support and insurance (and other topics as needed).
Communication Skills
1. Develop and maintain easy-to-understand print and multimedia materials and signage in the languages commonly used by the populations within the community. Communicate effectively both in written and verbal form.
2. Develop presentations, correspondence, and other communications that are well organized, easily understood, and effective. Communicate well with people of varied backgrounds. Elicit feedback and listen carefully to what others have to say.
3. Provide programs to community members and partners including giving presentations and public speaking.
4. Maintain confidentiality of client information as required by State and Federal laws and regulations and professional practice standards.
Health Equity and Trauma Informed Practice
1. Offer language assistance to individuals who have limited English proficiency and/or other communication needs.
2. Inform all individuals of the availability of language assistance services clearly and in their preferred language, verbally and in writing.
3. Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.
4. Collaborate with the community to identify systemic and structural barriers that perpetuate health inequities (e.g., discriminatory policies and practices, lack of affordable housing or public transportation, food deserts) and determine strategies to address these barriers.
Community Partnership Skills
1. Promote community action and garner support and resources from community organizations to implement new activities.
2. Conduct outreach, interpreting, translation and education for identified populations.
3. Participate in the planning and staffing of public outreach events.
4. Identify possible outreach strategies and locations to best reach target populations.
5. Act as a liaison between the Sheboygan County Division of Public Health and community agencies to facilitate communication and coordinate services.
6. Conduct outreach to recruit community members who experience barriers and connect them to resources such as insurance, transportation, housing, healthcare etc.
Public Health Sciences
1. Understand performance management and program performance measures, and identify opportunities for quality improvement.
Management and Finance Skills
1. Contribute to the development, implementation of work plans within assigned programs, organizational teams, and community health improvement initiatives.
Leadership and Systems Thinking
1. Participate in and apply quality improvement (QI) and performance management (PM) processes and/or techniques to improve the effectiveness of the respective public health program.
Other
1. Perform emergency response duties in alignment with the 15 CDC Public Health Emergency Preparedness Capabilities as assigned and consistent with job classification and training provided, in response to threats to the public's health.
2. Perform other duties as assigned.
Qualifications
- High school graduation and prior work, volunteer or lived experience in a human services organization required. Associate or bachelor's degree in a human services related field preferred.
- Successful completion of a written and oral language test with an above average rating.
- Knowledge of department and community programs, resources, services and eligibility criteria.
- Ability to establish rapport with persons from a variety of socio-economic, cultural and racial backgrounds.
- Ability to make clear and concise written and oral reports.
- Ability to develop and maintain cooperative working relationships with staff, supervisors, and consumers.
- Ability to learn and comply with department regulations.
- Ability to follow instructions and complete assigned tasks.
- Ability to maintain confidentiality.
- Ability to report to work or an alternate work site to provide continuity of services and assistance during community emergencies during regular and after work hours.
- Knowledge of or experience with trauma-informed principles and practices is preferred.
- A valid driver's license and ability to drive or arrange travel in and out of the county, as needed, to perform job duties.
Work Environment & Physical Requirements
WORKING RELATIONSHIPS
- Reports to Human Services Manager. Works with Public Health Professionals (Nutritionists, Sanitarians, Public Health Educator, Public Health Nurses), Human Services Assistants (Public Health Aide, Public Health Technician, WIC Assistant). Frequent contact with co-workers, consumers, members of the public, and representatives of other agencies.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS
Work is performed in a variety of office, home and community based settings. Ability to frequently lift up to 25 pounds and a combination of sitting and standing postures may be required. Ability to regularly work 8 hours per day and 40 hours per week is required. Hours will typically occur during standard business days but may vary in response to consumer needs and special events.
Must be able to effectively operate standard office equipment including telephone, computer, facsimile, calculator, printer, and photocopier.
Risk of blood borne pathogen exposure is considered low for all, but high for Public Health Technicians.
