We are hiring full time Registered Nurse to join our growing team in Rock Hill and surrounding areas by providing Case Management services to patients in their homes, assisted living and long-term care facilities who are enrolled in our hospice program.
***This is an overnight PRN position working Monday-Thursday 4:00 pm- 8:30 am ***
Responsibilities:
- Looking for someone with a Hospice heart and passion for providing end of life care
- Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG
- Responsible for overseeing patient plan of care with the collaborating physician
- Will conduct start of care visits and collaborate with Care Navigators
- Provide education to patients, families, and facility staff
Qualifications:
- Must have a RN license unincumbered in the state you are applying
- Reliable transportation and valid drivers license, clean MVR
- Experience in Hospice/Home Health is preferred
