Our caregivers and office staff must pass a background check and drug screenings throughout employment.
Role Overview:
As a Client Care Coordinator at Liberty Provider DBA Visiting Angels, you will be responsible for overseeing client care and maintaining relationships with clients and their families. In this role, you'll collaborate closely with our clients family to establish and maintain care for seniors in our community. The Client care coordinator is also responsible for working closely with our caregiving team to ensure we are maintaining high-quality care for our clients. Working on-site in Monroe, Michigan, you'll have the opportunity to support our mission of providing Liberty and Dignity through Service.
Key Responsibilities:
- Coordinate and establish care services for clients, ensuring their individual needs are met.
- Assist in the recruitment, training, and supervision of caregivers.
- Conduct assessments and develop care plans tailored to each individual client.
- Conduct supervisory visits to ensure we are providing excellent care.
- Maintain effective communication with clients, families, and staff to address concerns and feedback.
- Ensure compliance with all regulatory and company standards related to client care.
- Assist with maintaining a positive relationship with the Veterans Administration to allow us to continue serving those that once served our country.
Qualifications:
You're a great fit if you have the following skills.
- Previous experience in a supervisory or coordination role within healthcare or homecare.
- Strong communication and interpersonal skills.
- Ability to develop and maintain care plans based on client assessments.
- Proficiency in using technology for scheduling and client management.
- Compassionate approach to service and care for clients.
- Self driven and motivated to assist with the growth of the company.
Bonus If You:
- Have experience with senior care or familiarity with the specific needs of the elderly.
- Possess certifications related to care coordination or patient management.
- Are bilingual, with proficiency in languages beyond English.
Employment Type & Availability:
This position is Full-Time, Monday through Friday from 8am to 4pm with on call rotation.
Location:
This position is on-site in Monroe, Michigan and is open to applicants from Michigan and Ohio.
