Job Summary

We are seeking a compassionate and dependable Home Health Aide (HHA) with a (CNA) to provide in-home care for patients receiving home health and hospice services. This role focuses on personal care, comfort, safety, and emotional support for patients in their homes.

Responsibilities

  • Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility
  • Provide comfort care and support for hospice patients
  • Monitor and report changes in patient condition to the nursing team
  • Assist with light housekeeping related to patient care
  • Support safe transfers and ambulation
  • Provide companionship and emotional support to patients and families
  • Follow care plans and agency policies
  • Accurately document care provided

Qualifications

  • Current CNA license (per state requirements)
  • Experience in home health, hospice, long-term care, or caregiving preferred
  • Compassionate, patient-centered approach to care
  • Reliable transportation and valid driver's license
  • Ability to work independently in a home setting
  • CPR certification (or ability to obtain)
