Summary of Duties

  • Observe, assess (within scope of practice), and reports Patient conditions/changes to family and provider(s).
  • Provides necessary documentation in the EMR of all information relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
  • Assist in room changes and admissions.
  • Document appropriate happenings in all designated areas (nursing notes, incident reports, care plans, 24-hour report, etc.).
  • Ensure the health, comfort, and safety of patients by assisting with bathing, feeding, and dressing.
  • Consults with Director of Care and/or Administrator regarding changes in Patient's condition.
  • Ensures that each Patient's attending physician and family or responsible party is promptly notified of any significant change in the Patient's health condition.
  • Carry out the service plan as directed and administer medications and treatments as prescribed.
  • Responsible for assuring resident safety, including the delivery of appropriate clinical care by the CNA staff.

Qualifications

  • Must be a graduate of an approved school for practical or vocational nursing.
  • Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
  • Experience in rehabilitative or geriatric nursing, preferred.
  • Must maintain current BLS/CPR certification, preferred.

Pay: $29.00 per hour
