Efforts of this position will focus on support for hospitals systems participating in the HOC initiative. The coordinator will assist hospitals in implementation, technical assistance, administrative, and evaluation of HOC Initiative objectives.Objectives of the HOC Initiative are:o Assist in developing hospital and/or clinic workflow for supporting patient health-related social needs (HRSN)o Refer youth nicotine users to My Life My Quit (youth cessation resource)o Refer patients with BMI 30< to lifestyle weight and chronic disease management programso Refer patients or engage community members based on specific community wellbeing prioritieso Assist in implementation of telestroke serviceso Gather, analyze, maintain, report, and present data from hospital partners for HOC activitiesEssential Functions:o Assist in recruitment of hospitals to participate in the HOC Initiative.o Gain executive hospital leadership support and formal agreements for participation in the HOC Initiativea) Formal contract for participation and employment of a community health worker (CHW) and data sharing of CHW activitiesb) Formal contract for telestroke services and data sharingo Collaborate with hospitals and clinics to assist key staff in implementation of clinical workflows for the HOC Initiativeo Provide regular, systematic analysis of the operations, performance, and efforts to improve outcomes in the HOC Initiative.o Plan and convene meetings and trainings for hospital and clinic staff related to the quality improvement, evaluation, and technical assistance.o Regularly analyze and assist with data gathering and evaluation of HOC Initiativeo Collaborate with other OHA health improvement staff, O.U. Evaluators and TSET Program Officer on initiative evaluation.o Coordinate initiatives' efforts with other health improvement organizations including, but not limited to: Oklahoma State Department of Health, Oklahoma Health Care Authority, Oklahoma Department of Mental Health and Substance Abuse Services, Oklahoma Physician Manpower Training Commission, Oklahoma City-County Health Department, and the Oklahoma Tobacco Settlement Endowment Trust.o Collaborate with communications and marketing staff:a. within health systems to assure effective promotion of policies and procedures through multiple communication venues to patients, employees, and visitors.b. within the OHA and TSET communications/marketing staff to assure updated communication to OHA hospital members and TSET audiences of initiative news items.o Regularly review emerging scientific literature; attend trainings and professional development activities to stay current with systems change strategies to enhance employee wellbeing, address healthcare provider burnout, and obesity.Qualifications:Minimum bachelor's degree required in a health-related field including, but not limited to, nursing, social work, public health, community health or other health related field.o Minimum of 3 years of experience working in health-related field.Understanding of sustainable, system change principles and implementation science.Knowledge of the organizational complexities and infrastructure of hospitals and systems of healthcare.Prefer individuals with knowledge of and experience in:a. Public/population health principles and strategic implementation of objectivesb. Underserved and hard to reach populationsc. Community health work, or community health workersd.
