Job Summary:

SN Homehealthcare is seeking seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at SN Home Health Care. The LPN will provide high-quality, patient-centered care to individuals in their homes, ensuring their medical needs are met while promoting their comfort, dignity, and independence. The ideal candidate will have strong clinical skills, excellent communication abilities, and a dedication to improving patients' quality of life.

Key Responsibilities:

  • Provide direct nursing care to patients in their homes in accordance with the care plan established by the supervising Registered Nurse (RN) or physician.

  • Administer medications, wound care, injections, and other prescribed treatments.

  • Monitor patients' vital signs and overall health, documenting observations and reporting any significant changes to the care team.

  • Collaborate with RNs, therapists, and other healthcare professionals to deliver a coordinated plan of care.

  • Educate patients and their families on proper care techniques, medication management, and disease prevention.

  • Assist with activities of daily living (ADLs) as needed, such as bathing, dressing, and mobility support.

  • Maintain accurate and timely documentation of care provided, in compliance with agency and regulatory standards.

  • Follow all infection control procedures and promote a safe, clean environment for patients.

  • Participate in regular team meetings and in-service training to stay updated on best practices and protocols.

Job Qualifications

Qualifications:

  • Current and valid LPN license in Florida.

  • Minimum of 2 years of nursing experience, preferably in home health or a similar setting.

  • Current CPR and First Aid certification.

  • Strong clinical and critical thinking skills.

  • Excellent interpersonal and communication skills to build rapport with patients and families.

  • Ability to work independently and manage a flexible schedule.

  • Reliable transportation and a valid driver's license.

  • Proficiency with electronic medical record (EMR) systems is a plus.

Job Types: Part-time, PRN, Per diem

Pay: $ $35.00 per day
