The Center for Human Development, (CHD) is seeking a Caseworker for our Congregate Care Network (CCN) located in Springfield, MA. The Caseworker will be assigned to a Community Treatment Residence (CTR) and a Youth and Young Adult Group Residence (YYAGR). The program is for males ages 16-21 years old. Here they'll have access to the community to practice independent living skills.

YOUR ROLE AS A CASEWORKER:

The caseworker/manager is responsible for coordination of staff education around individual service plan goals, assist youth in making community linkages and mastery in independence skills. They are a liaison between the program, youth, families and external stakeholders to ensure an encapsulation of services. They maintain open communication, actively supports the team model and ensures all agency standards, policies, and procedures are utilized. 

REQUIRMENTS:

  • Experience with adolescents
  • Familiarity with residential programs (1-5 years experience)
  • Casework experience
  • Bachelor's degree in social work, counseling, psychology or related clinical field
  • Driver's license required

ESSENTIAL JOB FUNCTIONS:

  • Participates in team approach, sharing responsibility for clients in placement
  • Delivers services to any youth in the program in the event that assigned staff is unavailable
  • Coordination of services and treatments delivered by community-based provider
  • Assist youth in making community linkages ad mastery in independence skills
  • Liaising with schools, ABA providers, medical providers, as well as maintaining the ongoing regular communication with the department and other stakeholders
  • Include involvement of youth in the processes as appropriate
