Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including inpatient, outpatient, hospital, home health, and community care. This role involves developing and implementing individualized care plans to improve patient mobility, function, and overall quality of life. The Physical Therapist will utilize their expertise in physiology, anatomy, and medical terminology to deliver effective physiotherapy interventions tailored to diverse patient populations such as pediatrics, geriatrics, orthopedics, and those recovering from acute care or hospital stays.
Duties
- Conduct thorough assessments of patients' physical conditions, including evaluating mobility, strength, and functional limitations.
- Develop personalized care plans incorporating discharge planning, therapeutic exercises, manual therapy techniques such as trigger point therapy, and other modalities.
- Implement evidence-based physiotherapy interventions for patients in inpatient, outpatient, home & community care settings.
- Manage patient progress through regular re-evaluations and modify treatment plans accordingly.
- Educate patients and their families on injury prevention, home exercise programs, and health maintenance strategies.
- Document all patient interactions accurately using EMR systems while ensuring compliance with healthcare regulations.
- Collaborate with multidisciplinary teams to coordinate patient care and facilitate smooth transitions across care settings.
- Assist in training staff or students when applicable and participate in quality improvement initiatives related to patient care.
Experience
- Proven experience working in inpatient hospital environments, outpatient clinics, or home health settings is preferred.
- Demonstrated expertise in acute care management and outpatient physiotherapy services.
- Familiarity with care plans for diverse populations including pediatrics and geriatrics.
- Knowledge of physiology, anatomy, medical terminology, and orthopedic rehabilitation techniques such as trigger point therapy.
- Experience with EMR systems for documentation and patient record management.
- Prior exposure to discharge planning processes and working within hospital or community-based care frameworks is advantageous.
