Job Summary

We are seeking compassionate and dedicated Direct Care Workers to provide high-quality support and assistance to individuals with diverse needs, including seniors, individuals with developmental disabilities, and those requiring specialized care such as Alzheimer's, dementia, or hospice care. The ideal candidate will play a vital role in promoting independence, safety, and well-being through personalized care in various settings such as assisted living facilities, nursing homes, or home care environments. Duties

  • Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
  • Operate and document using Electronic Medical Record (EMR) systems to ensure accurate patient or resident information.
  • Administer medications following prescribed care plans and medication administration protocols.
  • Monitor vital signs such as blood pressure, pulse, and temperature; report any abnormalities to healthcare professionals.
  • Support residents with meal preparation and feeding as needed, ensuring dietary restrictions are followed.
  • Utilize assistive devices such as Hoyer lifts for safe patient transfers and mobility assistance.
  • Maintain cleanliness of the care environment through light cleaning duties to promote hygiene and safety.
  • Provide emotional support and companionship to residents with memory care needs or behavioral challenges related to dementia or autism.
  • Document observations related to behavior management, conflict resolution, and progress notes for case management purposes.
  • Support residents' participation in social activities and engagement programs to enhance quality of life.

Requirements

  • Prior experience in assisted living, nursing homes, home care, or working with individuals with disabilities is preferred.
  • Knowledge of ADLs assistance, Alzheimer's/dementia/memory care, hospice care, and behavioral management strategies.
  • Certification or training in First Aid and CPR is highly desirable; willingness to obtain certifications if not already held.
  • Familiarity with EMR systems, medical terminology, HIPAA regulations, and medication administration protocols.
  • Ability to lift heavy objects safely (heavy lifting) and assist with transfers using devices like Hoyer lifts.
  • Valid driver's license preferred for driving residents or clients when necessary.
  • Strong communication skills with the ability to work effectively with residents, families, and healthcare teams.
  • Compassionate attitude with a commitment to providing respectful and person-centered care.
  • Flexibility to work various shifts including days; some positions may require working weekends or holidays based on the facility's needs. This role is essential for supporting the health and happiness of individuals requiring assistance due to age, disability, or health conditions.
