We are currently seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our Modesto team.

Position Summary:

The Home Health LVN will provide skilled nursing care to patients in their homes. This includes wound care, medication administration, disease management education, and ongoing patient assessment. This position is ideal for LVNs who enjoy autonomy, flexibility, and meaningful one-on-one patient care.

Key Responsibilities:

  • Provide in-home nursing care under the direction of an RN Case Manager
  • Perform wound care, dressing changes, injections, and medication management
  • Monitor and document patient conditions, vital signs, and responses to treatment
  • Educate patients and families on disease processes, care plans, and home safety
  • Report significant changes in patient condition to RN Case Manager and physicians
  • Collaborate with the interdisciplinary care team
  • Maintain accurate and timely documentation
  • Travel throughout Modesto and surrounding areas for patient visits

Qualifications:

  • Active California LVN license
  • Home health experience preferred but not required (training available)
  • Strong clinical assessment and judgment skills
  • Excellent communication and documentation abilities
  • Reliable transportation and valid driver's license
  • Compassionate, dependable, and patient-focused
  • Must be able to pass a background check

Job Types: Full-time, Part-time, PRN, Per diem

Pay: $ $65.00 per hour
