About Us:

Creative Home Health, a division of Florry Creative Care Corp., provides compassionate, patient-centered home health services throughout Central Florida. We are seeking a skilled and motivated Physical Therapist (PT) to provide in-home therapy to our clients on an as-needed basis.

This position offers the flexibility of per-visit scheduling while maintaining employee status (W-2) — including applicable payroll tax contributions and agency support.

Position Summary:

The Physical Therapist will evaluate and treat patients in their homes, focusing on improving mobility, restoring function, and promoting overall independence. The PT will also provide education to patients and caregivers to support long-term outcomes.

This role includes a monthly stipend to ensure ongoing readiness, documentation oversight, and participation in agency activities, plus an hourly rate for direct patient visits.

Compensation Structure:

    • Monthly Stipend: $300— paid to retain availability and support administrative readiness (meetings, documentation follow-up, etc.)
  • Hourly Rate: $45–$55/hour for direct patient care
  • Mileage Reimbursement: Provided per agency policy
  • Flexible work schedule — hours based on patient assignments
  • Bimonthly payroll with standard tax withholdings

Responsibilities:

    • Perform comprehensive physical therapy evaluations and develop individualized treatment plans
  • Deliver skilled therapy interventions in accordance with physician orders and agency policies
  • Complete timely and accurate clinical documentation in the EMR
  • Communicate regularly with nursing, case management, and other interdisciplinary team members
  • Participate in care conferences or QA meetings as needed (covered under stipend)
  • Maintain current licensure and CEU requirements per Florida law

Qualifications:

    • Active Florida Physical Therapist License (required)
  • Minimum of 1 year of PT experience, preferably in home health or geriatrics
  • Current CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
  • Strong communication and time-management skills
  • Commitment to patient-centered, compassionate care
