CLTS CASE MANAGER
The Washington County Human Service Department, located in West Bend, Wisconsin, has an excellent opportunity as a full-time CLTS Case Manager. The Washington County Human Services Department offers responsive and effective community programs to individuals and families challenged by disability, economic hardship, and safety concerns. Under the general direction of the Youth Treatment Team Supervisor, the Case Manager is responsible for the planning, delivery, and coordination of case management services for children with disabilities and their families.
Qualified candidates must possess:
- Bachelor's Degree in a health or human services related field.
- A minimum of two years of professional work experience preferably working with children with disabilities and their families in community programs.
- Experience in waiver programs preferred.
- Must possess a valid Wisconsin driver's license.
Starting hourly rate is $28.21- $37.50 based on experience.

Position Summary:
Under the general direction of the Youth Treatment Team Supervisor, the CLTS Case Manager is responsible for the planning, delivery, and coordination of case management services for children with disabilities and their families.
Duties/Responsibilities:
The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
- Comply with requirements outlined in Medical Assistance (MA) Waiver Manual, Third Party Administration billing, Children's Community Options Program.
- Completes assessments as outlined for the Children's Functional Screen with all required resources information gathered. This includes annual recertification for the Children's Long Term Support Programs.
- Develop individual service plans to meet children's needs defined in outcomes, while complying with the requirements as outlined in MA Waiver Manual and Children's Community Options Program. This includes ensuring the child's health and safety with stated outcomes.
- Obtain proposals for services within the guidelines of the CLTS Waiver allowable services, authorize and monitor these services and provider agreements.
- Serve as a liaison between clients, families, service providers, and various state offices.
- Allocate funding based on client needs defined within the Assessment and Individual Service Plan.
- Maintain required contact lines with the clients and families as per MA Waiver Manual and Children's Community Options Program.
- Strategize with families and providers to address issues and concerns, and provide assistance and support.
- Maintain the clients community living arrangements, preferably within their parental home whenever possible.
- Maintain the case file for each client on the caseload, this includes meeting time limit standards per the Human Services Department.
- Provide relevant CLTS Waiver and Children's Community Options Program updates at the quarterly Children's Advisory Committee meeting.
- Work within a team environment with Human Services staff, community members, school personnel, provider agencies and DHS staff.
- Performs other duties as assigned, including responding to an emergency event.
Work Direction Received:
Works under the general direction of the Youth Treatment Team Supervisor.
Decision Making:
Decisions concerning service authorization, residential placement, crisis intervention, treatment plans, and daily activities are made independently.
Interaction:
There is significant interaction with various provider agencies, community organizations, other County Departments, clients, families, guardians, and the general public.
Essential Knowledge and Abilities:
Knowledge of systems and programs enumerated in Chapters 51.42, and 48 of Wisconsin Statutes, services available in the community for children with disabilities and their families, and applicable State and Federal laws and regulations. Ability to ascertain client needs, communicate effectively orally and in writing, and interact effectively with clients, families, other health care providers and the general public. Able to fulfill physical demands of job, sensory demands (seeing and hearing) and cognitive demands (concentration, conceptualization, memorization).
Training and Experience:
Bachelor's Degree in a health or human services related field and a minimum of two years of professional work experience preferably working with children with disabilities and their families in community programs. Experience in waiver programs preferred. Must possess a valid Wisconsin driver's license.
