HEALTH AND WELLNESS CASE MANAGER

SUMMARY: Health and Wellness Case Managers are essential contributors to the care, coordination, and advocacy of residents and day students. This role involves close collaboration with interdisciplinary teams, internal and external stakeholders, and family/guardians to ensure students' healthcare needs are efficiently and effectively met. Responsibilities include maintaining accurate medical records, coordinating and attending medical appointments, training new staff, and facilitating communication with parents and staff. Successful candidates will demonstrate attention to detail, strong organizational and communication skills, and a commitment to promoting student health and well-being. This position offers the opportunity to work in a supportive and collaborative environment, contributing to meaningful outcomes for students and their families.

REQUIRED EDUCATION & EXPERIENCE:

  • Associate degree in nursing or a bachelor's degree in social or behavioral sciences, such as nursing, social work, psychology, or sociology.
  • Preferred: Experience working with individuals with developmental disabilities.
  • Preferred: Case management experience or relevant training.
  • Ability to obtain First Aid and CPR certification.
  • Ability to complete Community-Based Residential Facility (CBRF) Medication Administration certification
  • Strong verbal and written communication skills, as well as interpersonal skills and a passion for providing inclusive care.
  • Proficiency in general office skills, including Microsoft Outlook and Word, as well as a demonstrated ability to use or learn Electronic Medical Record (EMR) systems.
  • Knowledge and application of Trauma-Informed Care principles.

GENERAL HOURS OF DUTY:

Full-time, primarily 1st shift hours, Monday through Friday with flexibility to meet the needs of the students served at Chileda.

JOB DUTIES:

  • Collaborate with the interdisciplinary team, including Case Managers, to ensure students' health care needs are integrated into treatment goals and plans.
  • Maintain accurate, thorough, and up-to-date medical records, including medical charts, daily logs, electronic medication administration records, and client management systems.
  • Provide guidance and instruction to students in developing life skills related to their health care needs.
  • Advocate for the health and well-being of students.
  • Communicate professionally with internal and external stakeholders related to the health needs and updates of the students.
  • Provide first aid during emergencies and facilitate access to medical attention as necessary.
  • Observe students during illnesses, consult with medical professionals, and implement recommended treatments.
  • Train and mentor new Health and Wellness Care Professionals under the guidance of the Health and Wellness Manager.
  • Collaborate with Case Managers for treatment goals related to Health and Wellness and provide progress towards those goals.
  • Complete treatment plans and attend quarterly and IEP meetings per Case Manager request, and ensure staff are updated on assigned student's progress.
  • Attend pre-enrollment meetings for new admissions.
  • Complete admission and discharge paperwork for residential and day students as , ensuring all required paperwork is accurate and submitted promptly.
