ChristianaCare is seeking an enthusiastic, compassionate, and adaptable Nurse Practitioner or Physician Assistant to join our growing Mobile Health Services and Medical Respite Program at the New Castle County Hope Center. About the Role

In this unique and rewarding position, you will split your time between two vital programs serving the Wilmington community, with opportunities to practice women's health, pediatric care, and substance use disorder treatment in both settings.

Mobile Health Services

Our Mobile Health Services van brings comprehensive primary care — and elements of street medicine — directly to individuals and families throughout Wilmington. You'll deliver a broad range of services including:

  • Wellness visits, sports physicals, and pediatric primary care
  • Women's health services (including preventive and reproductive health care)
  • Chronic disease management
  • Wound care, vaccinations, and sick visits
  • Support for patients navigating substance use disorders, including medication-assisted treatment pathways (ie buprenorphine, vivitriol), harm-reduction education, and continuity of care connections

You'll rotate through community center locations, providing both walk-in and scheduled visits — offering autonomy in your clinical decision-making and the opportunity to build meaningful, long-term relationships across the community.

Medical Respite Program

Located at the New Castle County Hope Center, the Medical Respite Program provides a safe, supportive environment for individuals facing medical or behavioral health challenges, often compounded by social or housing instability. You'll work within a collaborative, multidisciplinary team to support patients with:

  • Acute and chronic medical needs
  • Behavioral health concerns
  • Social and housing instability

This setting allows for a high degree of independent practice while still benefiting from close collaboration with an engaged, mission-driven care team.

Qualifications

  • Master's degree in Nursing or Physician Assistant Studies
  • Delaware license or license-eligible
  • FNP certification required
  • Minimum 1 year of provider experience preferred
  • Prescriptive Authority, DEA registration, and BLS required

Annual Compensation Range $112, $147,511.00This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
