Counselor I: LSW/LISW, LPC/LPCC
Role Summary
At Midwest Integrated Care (Midwest), Counselor I serves as a member of a comprehensive healthcare team that includes primary care, behavioral health, and care coordination providers serving a diverse patient population in a fast-paced primary care setting. Contributing to a supportive work environment in a healthcare setting with a patient-centered approach, Counselor I is responsible for providing assessments, screenings, consultation, and ongoing treatment/counseling to assist patients in managing mental health symptoms and underlying stressors.
Key Responsibilities
Care Planning, Counseling, & Coordination
- Responsible for the assessment, treatment planning, and episodes of therapy for established patients in collaboration with the care coordinator.
- Demonstrates a working knowledge of and ability to practice evidence-based therapies (CBT, DBT, Motivational Interviewing, etc.).
- Responsible for brief intervention counseling as referred by medical providers. Acts as a consultant to other staff on skillful and effective intervention with patients, including crisis intervention.
- Acts as a consultant and resource to members of the care team on skillful and effective interventions with patients.
- Demonstrates knowledge of regular patient screenings (PHQ-9, GAD, etc.) and effectively provides necessary services.
- Monitors patient adherence to care plans, identifies barriers preventing adherence to care plans, and works collaboratively with patients to manage and/or eliminate these barriers.
- Ability to communicate effectively with various levels of the care team across all integrated care team disciplines.
Compliance & Documentation
- Actively participates in professional development activities that support the integrated behavioral health program.
- Educates and assists the primary care team in developing guidelines and education for patients.
- Assists with staff training as needed.
- Maintains familiarity with current DSM and ICD diagnosis codes.
- Completes all clinical documentation, including, but not limited to, assessments, care plans, and session notes in a timely manner.
- Ensures documentation meets Joint Commission, HIPAA, and 42 CFR Part 2 requirements.
Outcome Tracking & Advocacy
- Monitors patient progress and collaboratively updates care plans as needed.
- Tracks patient follow-up and clinical outcomes via the EHR for value-based care initiatives.
- Advocates for patient rights, safety, and access to services.
Qualifications
- Required Education/Experience: Master of Social Work or related degree from an accredited program, with corresponding license (e.g., LISW, or LSW in pursuit of LISW)
- Preferred Experience: Two years of experience in behavioral health and/or substance use disorder counseling.
- Ability to maintain effective and professional relationships with the patient and other members of the care team.
- Working knowledge of diagnosis of common mental health and/or substance use disorders.
- Experience with assessment and treatment planning for common mental health and/or substance use disorders.
- Effective communication, organizational, collaboration, and documentation skills.
- Knowledge of integrated behavioral health models and interventions.
- Able to effectively communicate through verbal/written expression.
