Job Summary for Value Based Medicine Specialist: The Value Based Medicine Specialist is an expert in value based primary care measures, care gap outreach and closure guidelines. This role is responsible for phone outreach to patients for care gap management, medication adherence, suspect condition assessment, and communicating critical information to stakeholders. The ideal candidate must be very comfortable with outbound phone communication.

Qualifications for a Value Based Medicine Specialist:

  • High school diploma or equivalent
  • 2(+) years Medical Assistant Experience OR Care Gap Closures
  • 1(+) year(s) of experience in a value-based primary care setting managing care gap closure for large quantities of patients
  • Microsoft Excel and EMR experience proficiency required; (Epic EMR a plus, but not required)
  • Possess strong verbal and written communication skills
  • Excellent interpersonal and customer service skills
  • Bilingual in English and Spanish preferred

Job Type: Full-time

Pay: $ $25.00 per hour
