Job Summary

Supportive Personal Care Services, LLC is a Medicaid-certified home care agency expanding our Caregiver Pool in Milwaukee and surrounding areas.

We are seeking compassionate, dependable Personal Care Workers (PCWs) and Supportive Home Care (SHC) caregivers to provide non-medical home care services for adults, seniors, and children enrolled in Medicaid LTC Waiver programs, including CLTS (Children's Long-Term Support) and Family Care/IRIS Supportive Home Care.

As an independent contractor, you will have the freedom to create your own schedule while supporting clients in their homes with activities of daily living and supportive home care needs.

Responsibilities

Personal Care Services (PCS)

  • Provide personal care such as bathing, dressing, grooming, and toileting
  • Assist with safe transfers and mobility (bed, chair, wheelchair)
  • Perform light housekeeping tasks related to personal care
  • Prepare and serve meals; assist with grocery shopping as needed
  • Observe and report changes in condition to the supervising RN
  • Maintain accurate, timely service documentation

Supportive Home Care (SHC) – Adults & Children

  • Assist with basic household cleaning, organization, and sanitation
  • Provide supervision and safety monitoring as needed
  • Support daily routines for children and adults with physical, developmental, or behavioral needs
  • Assist with skill-building, social activities, and community outings when appropriate
  • Follow individualized service plans for members enrolled in LTC waiver programs (Family Care, IRIS, CLTS)

Qualifications

  • Must be at least 18 years old
  • Ability to read, write, and follow written care plans or support plans
  • Reliable transportation; valid driver's license preferred
  • Physically capable of caregiving tasks (lifting, bending, standing)
  • Professional, dependable, and respectful of client confidentiality
  • Must complete agency onboarding/orientation
  • At least 1 year of caregiving experience (PCW or SHC) is required

Job Type: Contract

Pay: $15.00 per hour

Expected hours: 15 – 30 per week
