ResponsibilitiesThe Program Coordinator will oversee the activities of program Care Managers to assure the provision of high-quality, family-driven and youth-guided service that is in accord with the policy and regulations of Toomey Residential and Community Services, lead Health Homes, and the NYS Department of Health. Support Care Managers in their efforts to help families and children advance the health care status of youth eligible for Health Home service. Direct and provide support to staff and provide appropriate guidelines. Review client charts monthly to ensure compliance with agency, Health Home, and DOH guidelines. Review care manager notes monthly to ensure delivery of high quality and billable services. Assist in recruiting, training and evaluating program staff. Maintain accountability for all assigned staff. Perform Outreach activities as necessary. Provide comprehensive care management, care coordination, comprehensive transitional care, youth & family support & health promotion. Provide comprehensive assessment and reassessment of youth as needed inclusive of medical, behavioral, rehabilitation, social service and long term care needs. Prepare crisis intervention plan and participate in its execution as needed.
QualificationsQualifications - A Bachelor's Degree or RN with two (2) years of relevant experience, or a Master's Degree with one (1) year of relevant experience. Master's of Social Work and knowledge of Health Homes programs preferred. Two years supervisory experience preferred. Valid N.Y.S. Driver's License, NYS driver's license check, criminal background check, State Central Register Clearance.
