Drug and Alcohol Case Manager

The position requires an individual who possesses the competencies, knowledge, skills, and attitudes essential to the effective practice of case management for substance use disorders. The individual needs to be fully proficient in understanding addiction, treatment, and recovery, and able to practice professional ethics. They must also become fully proficient in the practice of Recovery-Oriented Systems of Care (ROSC) and the utilization of the Social Model Approach to support, planning, referral, and service coordination.
JOB STATUS: Full-Time Non-Exempt
PRIMARY FUNCTIONS:
1. Works in collaboration with clients to develop their recovery plan within 24 hours, assisting residents
meet their goals of the recovery plan, following up with resident progress on a weekly basis, and updating the recovery plan at least every 30 days
Collaborates with community organizations for resources, referrals and service delivery; create and maintain an updated resource guide of available recovery supports; maintain updated housing lists, seek out employment opportunities for those with a criminal history, and offer resources to other unmet needs required to fulfill the goals of the recovery plan (ex. GED, Driver's license restoration, state ID, etc.).
Provides screening and intake to determine if a further assessment/evaluation or higher level of Care is needed
Follow up with the client to confirm access to the appropriate level of care. Upon consent, follow up with the referral source to ensure a smooth transition to intervention/treatment
Utilizes effective retention techniques and evidence-based approaches to maintain participation, such as motivational interviewing techniques, and will incorporate ROSC principles throughout the process.
Organizes and conducts social and cultural activities to promote sobriety and awareness about substance abuse.
Utilize the Social Model approach to recovery
Maintains confidential participant records and/or data. Completes monthly, quarterly, and annual reports in a timely manner.
Performs other related duties and other duties as requested and/or necessary.
OTHER RESPONSIBILITIES:
1. Maintains required training hours and clearances.
Maintains open communication with the supervisor and other staff as it relates to the necessary flow of the job and programs. Aids in the design of educational programs.
Continually remains up to date on the individual, group, and family counseling techniques and evidence-based approaches. Possesses and utilizes the ability to relate to problems and the special needs of clients.
Maintains community resources to make them available to the clients.
Must possess and utilize excellent verbal and written communication skills and general computer skills, including WORD, EXCEL, Gmail, internet, and other systems.
Must have an understanding of the criminal justice system, ATOD pharmacology, confidentiality, FERPA and PPR laws and regulations, and DDAP Best Practices Guidelines.
WORKING CONDITIONS:
While performing the duties of this job, the employee is regularly required to talk and hear. The employee is frequently required to stand, sit, and walk; use hands; and reach with hands and arms. The employee must be capable of lifting or pulling 175 lbs. Specific vision abilities required by this job include close vision and the ability to adjust and focus. Proper diction and command of the English language are required.
The Case Manager, preferable will have lived experience with substance use disorder, at least 18-24 months of sustained recovery; Master's Degree preferred; Bachelor's Degree in social work, psychology, criminal justice, social science or other related fields, and a minimum of 2 years of experience working with adults or a CADC and minimum of 3 years of relevant experience. Knowledge of the criminal juvenile justice system, the justice system, adult probation, and parole is essential. Reliable transportation and a valid Pennsylvania driver's license, as well as current PA auto insurance, are required.
All required clearances must be current, including Act 33/34 Child Abuse Clearance, Pennsylvania Criminal History Clearance, and FBI Clearance.
