Physician - Skilled Nursing Facility (SNF)
Job Summary:
The Physician - Skilled Nursing Facility (SNF) provides comprehensive medical care to residents within assigned skilled nursing facilities. This role focuses on delivering high-quality, patient-centered care, managing chronic conditions, addressing acute illnesses, and ensuring smooth transitions of care. The physician collaborates closely with interdisciplinary teams, including nurses, therapists, social workers, and administrative staff, to optimize resident health and well-being.
Responsibilities:
- Patient Care:
- Conduct comprehensive medical assessments and examinations of SNF residents.
- Develop and implement individualized treatment plans based on patient needs and goals.
- Manage chronic conditions, including diabetes, hypertension, heart disease, and dementia.
- Diagnose and treat acute illnesses and injuries.
- Perform routine physical examinations and preventative care.
- Order and interpret diagnostic tests, including laboratory studies and imaging.
- Prescribe and manage medications, ensuring appropriate dosage and monitoring for side effects.
- Provide palliative care and end-of-life support as needed.
- Respond to medical emergencies and provide timely interventions.
- Document all patient encounters and care provided in the electronic health record (EHR) accurately and completely.
- Collaboration and Communication:
- Collaborate with the interdisciplinary team to develop and implement care plans.
- Communicate effectively with residents, families, and caregivers regarding treatment plans and progress.
- Participate in care conferences and team meetings.
- Provide education and support to nursing staff regarding patient care and medical management.
- Communicate with referring physicians and specialists as needed.
- Communicate with pharmacies to ensure proper medication delivery.
- Compliance and Quality:
- Adhere to all federal, state, and local regulations and guidelines.
- Maintain accurate and up-to-date medical records.
- Participate in quality improvement initiatives and performance improvement activities.
- Ensure compliance with SNF policies and procedures.
- Follow all Medicare and Medicaid guidelines.
- Administrative Duties:
- Complete necessary documentation for billing and reimbursement.
- Manage patient schedules and appointments.
- Participate in medical staff meetings and committees as required.
- Complete all necessary paperwork in a timely manner.
Qualifications:
- Medical degree (MD or DO) from an accredited medical school.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Active and unrestricted medical license in the state of practice.
- Experience working in a skilled nursing facility or long-term care setting preferred.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Proficiency in using electronic health records (EHRs).
- Knowledge of Medicare and Medicaid regulations.
- Compassionate and patient-centered approach to care.
Preferred Qualifications:
- Certification in Geriatric Medicine.
- Experience with wound care and management.
- Experience with Palliative care.
Physical Requirements:
- Ability to stand, walk, and sit for extended periods.
- Ability to lift and move patients as needed.
- Ability to use standard medical equipment.
- Ability to travel between SNF locations as required.
Job Type: Full-time
Pay: $250, $350,000.00 per year
