Responsibilities include:

  • Provide personal care assistance (bathing, dressing, grooming, toileting, feeding, mobility support) to hospice patients in alignment with the care plan
  • Offer companionship, comfort, and emotional support to patients and their families
  • Assist with light household tasks directly related to patient care (changing linens, maintaining a safe and clean environment)
  • Report patient condition changes promptly to the supervising nurse
  • Document care provided in accordance with policies and regulations
  • Travel to patient homes/facilities as scheduled

Qualifications

  • Active CNA certification in the state of Texas
  • At least 1 year of CNA experience required (hospice experience preferred, but not required)
  • CPR licenses
  • Compassionate approach with excellent communication skills
  • Ability to travel within assigned area and manage schedule responsibly
  • Team player with a heart for serving patients and families
