Responsibilities include:
- Provide personal care assistance (bathing, dressing, grooming, toileting, feeding, mobility support) to hospice patients in alignment with the care plan
- Offer companionship, comfort, and emotional support to patients and their families
- Assist with light household tasks directly related to patient care (changing linens, maintaining a safe and clean environment)
- Report patient condition changes promptly to the supervising nurse
- Document care provided in accordance with policies and regulations
- Travel to patient homes/facilities as scheduled

Qualifications
- Active CNA certification in the state of Texas
- At least 1 year of CNA experience required (hospice experience preferred, but not required)
- CPR licenses
- Compassionate approach with excellent communication skills
- Ability to travel within assigned area and manage schedule responsibly
- Team player with a heart for serving patients and families
