Clinical Nurse/Patient Care Coordinator Job Opening

Core Family Practice is an outpatient direct primary care practice in Kennett Square and Glen Mills, PA that has been serving the community since 2016. We are a unique medical office that does not require insurance but rather runs on a monthly membership. Our physicians have smaller panels of patients (max 600 each) and provide more comprehensive, personal, and accessible care. We are a family practice that cares for all ages and we also work with businesses to care for their employees. We aim to create a clinical environment that is the opposite of the large health systems for both our patients and employees.

We are looking for a highly motivated, personable, and caring individual to be our clinical nurse. This position is for the lead nurse responsible for assisting the physicians with clinical care. It is important for any candidate to want to work as part of a team and provide excellent customer service.

Clinical Nurse/Patient Care Coordinator Responsibilities:

 Draw Labs/Perform EKGs/IVs/Admin meds/Allergy Shots/Immunizations

 Run in office tests – Urinalysis/Strep/Pregnancy tests

 Manage exam room and lab inventory

 Manage Quest/Lab Corp/MDL/Andameds/Cole Diagnostics/Vaxcare; work with reps

 Manage medication inventory/ordering

 Help with medication dispensing

 Manage patient medication refill requests

 Help patients set up cash pay radiology/specialist visits as needed

 Triage patients clinical question calls

 Cover nurse questions/triage/prescriptions at other Core office as needed

 Follow up calls with patients as needed

 Help follow up with our weight management/diabetic patients regarding their weight

loss/diet goals

 Occasional home lab draws/med administrations

 Manage biohazard waste

 Stay up to date on HIPPA/CLIA/OSHA compliance and determine our office requirements

 Assist with procedures as needed

 Assitance with Prior Authorizations

Qualifications and Requirements:

• LPN or RN Required
• Phlebotomy experience
• A minimum of 2 years of professional experience in a clinic or medical practice is preferred

Pay: From $32.97 per hour
