Nurse Practitioner (Licensed NP) for Mobile Wound Healing LLC an affiliate of Restore First Health as a member of the medical team that specializes in advanced wound care. Mobile Wound Healing LLC delivers advanced wound care services at patients' residences inclusive of their homes, Assisted Living Facilities, and Independent Living Facilities. This role involves providing care for patients by assessing and monitoring wounds and making clinical decisions while creating/maintaining a plan that results in successful wound care management in collaboration with home health and other health care providers. 

Responsibilities include:

  • Traveling to scheduled locations weekly for wound rounds at various homes or facilities in Columbus area
  • Providing the highest quality advanced wound care services to patients in homes and partner facilities
  • Performing high level bedside wound care skills including wound assessment, debridement, dressing application, pressure relief, pain/odor control, and minor surgical procedures
  • Tracking the progress of wound healing and documenting accordingly in EHR
  • Developing a treatment plan for wound care based on the individual needs of each patient 
  • Providing education to the patient, family, facility staff, and home health agency staff to promote understanding and healing, and for the management of patient's wounds

Experience:

  • Quality patient care
  • Desire to work with geriatric population
  • Valid unrestricted state license
  • Graduate degree in nursing
  • Valid driver's licenses and willingness to travel to facilities and homes
