Role Description

This is a full-time, on-site position for a Nurse Practitioner based in Oklahoma City, OK. The Nurse Practitioner will deliver comprehensive healthcare services, including conducting patient assessments, developing and implementing care plans, managing chronic conditions, and collaborating with an interdisciplinary team to coordinate care. The role involves providing education to patients and their families and ensuring high-quality, patient-centered care in alignment with the mission of Valir PACE.

Qualifications

  • Strong clinical skills, including patient assessment, diagnosis, and treatment planning for geriatric populations.
  • Experience in developing care plans and managing chronic medical conditions.
  • Ability to work collaboratively within a team-based healthcare model, including interdisciplinary collaboration.
  • Effective communication, patient education, and counseling skills.
  • Proficiency with electronic medical records (EMR) systems and documentation.
  • Current Nurse Practitioner License and certification (e.g., FNP, AGNP, or equivalent) with relevant state licensing requirements.
  • Prior experience working with elderly patients or in a PACE program is highly preferred.
  • Demonstrated commitment to patient-centered care and compassionate service.
