Essential Job Functions/Responsibilities:

  1. Leadership & Supervision: Direct and support clinical staff, including nurses and therapists, through coaching, mentoring, and performance evaluations.
  2. Care Coordination: Ensure timely initiation and delivery of services, monitor patient progress, and facilitate interdisciplinary collaboration.
  3. Quality Assurance: Review clinical documentation for accuracy and compliance; implement corrective actions as needed.
  4. Regulatory Compliance: Maintain adherence to state, federal, and accreditation standards; prepare for audits and surveys.
  5. Staff Development: Organize in-services, training sessions, and competency assessments to promote professional growth.
  6. Problem Resolution: Address clinical issues, patient concerns, and operational challenges promptly and effectively.
  7. Resource Management: Monitor staffing levels, productivity, and resource utilization to optimize care delivery.
  8. Participate in Meetings: Lead case conferences and contribute to organizational planning initiatives.
Qualifications
Position Qualifications:

  1. Current RN license in the state(s) of practice.
  2. Minimum of 2 years of clinical experience in home health; supervisory experience preferred.
  3. Strong knowledge of home health regulations and best practices.
  4. Current CPR certification by an organization-approved provider.
  5. Excellent communication, leadership, and organizational skills.
