Essential Job Functions/Responsibilities:
- Leadership & Supervision: Direct and support clinical staff, including nurses and therapists, through coaching, mentoring, and performance evaluations.
- Care Coordination: Ensure timely initiation and delivery of services, monitor patient progress, and facilitate interdisciplinary collaboration.
- Quality Assurance: Review clinical documentation for accuracy and compliance; implement corrective actions as needed.
- Regulatory Compliance: Maintain adherence to state, federal, and accreditation standards; prepare for audits and surveys.
- Staff Development: Organize in-services, training sessions, and competency assessments to promote professional growth.
- Problem Resolution: Address clinical issues, patient concerns, and operational challenges promptly and effectively.
- Resource Management: Monitor staffing levels, productivity, and resource utilization to optimize care delivery.
- Participate in Meetings: Lead case conferences and contribute to organizational planning initiatives.
Position Qualifications:
- Current RN license in the state(s) of practice.
- Minimum of 2 years of clinical experience in home health; supervisory experience preferred.
- Strong knowledge of home health regulations and best practices.
- Current CPR certification by an organization-approved provider.
- Excellent communication, leadership, and organizational skills.
