Clinical Case Manager/Counselor

The Clinical Case Manager will play a dual role in delivering virtual and in-person IOP counseling services at PBR, while also providing case management and AOD counseling services at AHR. This position requires someone who is flexible, organized, and passionate about supporting clients through recovery. This is a full-time position, with duties spanning both direct clinical care and supportive case management.
Key Responsibilities:
- Facilitate individual and group counseling sessions as part of the IOP (in-person and virtual).
- Develop and implement individualized treatment plans.
- Maintain up-to-date clinical documentation in compliance with program standards.
- Support clients in early recovery with education, emotional support, and relapse prevention tools.
- At AHR:
- Provide case management services, including connecting clients to community resources (housing, employment, medical care, etc.).
- Facilitate groups, Conduct assessments, monitor treatment progress, and assist with discharge planning.
- Deliver AOD (Alcohol and Other Drug) counseling services.
- Collaborate with multidisciplinary teams to ensure continuity of care.Qualifications:
- Registered or certified with a California board (AMFT, ACSW, APCC, CADC, or similar).
- Experience in substance use/mental health treatment settings.
- Strong understanding of co-occurring disorders, trauma-informed care, and recovery models.
- Comfortable working independently across multiple sites and platforms.
- Excellent communication, time management, and documentation skills.
- Proficient in virtual platforms (Zoom, EHR systems, etc.).Preferred:
- Familiarity with ASAM criteria
Competencies:
- · Ability to perform duties as a counselor (12 Core Functions). Competencies: with respect to screening, assessment, individualized treatment planning, crisis intervention, group and individual counseling, case management, referral, discharge summaries, clinical documentation, and professional ethics.
- · Ability to establish and maintain cooperative working relationships with supervisors, utilization review, managed health care clinicians, medical and mental health professionals.
- · Ability to positively interact and develop rapport with residents and their families.
Education:
- · Preferred AA or Bachelor's degree from an accredited college in the Human Services or Behavioral Health
- · Registered as an AOD intern or a RADT-1 Intern
- · Drug and Alcohol Counseling Certification Desired. (CCAPP,CAADE,CADTP or Affiliate)
Experience:
- · 1-2 years experience in the field of addiction and mental health or equivalent human services work experience preferred.
- · Experience in co-occurring disorders treatment a plus
Certificates & Licenses:
- · Valid Driver's License
- · CPR/First AID
- · Drug and Alcohol Counseling Credential or Internship Preferred
Other Requirements:
- · Negative TB test results within past year
- · Medical Clearance to work within past year
- · Ability to pass pre-employment and random drug screenings
Job Type: Full-time
