POSITION SUMMARY
APH is seeking an experienced and patient-focused Transition Specialist. As the Transition Specialist, you will be pivotal in optimizing patient care by developing, implementing, and managing transitional care for our members. Leveraging your expertise, you will ensure safe, effective, and appropriate support with healthcare teams to enhance patient outcomes and promote overall wellness.
This position requires travel in and around Flagstaff, Coconino County, and the surrounding areas to provide in-home and healthcare facility support to our patients.
DUTIES & RESPONSIBILITIES
- To assist clients in their return home from an acute inpatient stay.
- To increase communication with the client by building a face-to-face rapport before they are discharged from the facility to home.
- To capture and update demographic information.
- To help determine potential needs upon discharge.
- To perform in-home safety checks, collect data, and make recommendations to prevent readmissions.
- To build confidence in the care team.
- Demonstrate a positive and professional attitude in providing customer service by being respectful of all clients.
- Work collaboratively within a team setting to provide positive outcomes for clients.
- Participate in forming solutions for presenting problems and determining realistic goals.
- Demonstrate professional, appropriate, and effective communication with clients, families, and other team members to ensure continuity of care.
- Maintain patient confidentiality, following all HIPAA guidelines and regulations.
- You will be required to travel to your assigned areas including patient homes, hospitals, and acute care settings.
- You will also be expected to complete any other duties assigned by the Field Manager.
- Performs other related duties as assigned by management.
- One year of related healthcare experience.
- Proficient in Microsoft Office (Outlook, Word, Excel, and PowerPoint).
- Ability to work independently with minimal supervision or function in a team environment sharing responsibility, roles, and accountability.
- Must be a team player, organized, and able to handle multiple projects.
- Excellent oral and written communication skills.
- Strong interpersonal and organizational skills.
- Clean driving record.
- High school diploma or general education degree (GED) is required.
- Associate degree preferred.
- Certified Nursing Assistant, Medical Assistant, or Emergency Medical Technician certificate is highly preferred.

- Required to travel up to 1.5 hours one way.

- Must be able to physically perform the essential duties of the position which include lifting 30 lbs., transporting materials, stooping, kneeling, crouching, reaching, use of hands, balancing, walking, standing, talking, hearing, and typing.
