Job Summary

Tatyanas Case Management is seeking compassionate and reliable Caregivers to support clients under the Medicaid Home and Community Based (HCB) Waiver Program. The Caregiver Floater is responsible for ensuring continuity of care when a primary caregiver is unavailable, on leave, or when a new case requires temporary coverage. This position requires strong communication skills, attention to detail, and the ability to quickly learn each participant's individual care plan, preferences, and needs.

Key Duties and Responsibilities

  • Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.
  • Prepare and serve nutritious meals according to dietary needs and preferences.
  • Provide companionship and emotional support, promoting a positive and respectful environment.
  • Support client mobility and transportation, including assistance with walking, transfers, and attending appointments or outings.
  • Monitor client condition and promptly report changes or concerns to supervisors or assigned case managers.
  • Perform light housekeeping, laundry, and home organization to maintain a clean, safe living environment.
  • Follow and help implement goals outlined in the individualized Plan of Care.
  • Encourage client independence and participation in daily routines and activities.
  • Maintain accurate records of services provided and complete documentation per program requirements.

Qualifications

  • Prior caregiving experience in home care, long-term care, or group home settings is preferred.
  • Familiarity with Medicaid Waiver Programs or experience working with individuals with disabilities is a plus.
  • Basic understanding of medication reminders, safety protocols, and first aid.
  • Ability to prepare simple, healthy meals tailored to client needs.
  • Compassionate, dependable, and committed to preserving client dignity and autonomy.
  • Strong communication and interpersonal skills.
  • Must be able to work independently and manage time effectively.
  • Valid driver's license and reliable transportation preferred for client-related travel.
  • CPR/First Aid certification preferred; training will be provided.
  • TB Test preferred; will be provided.
