Job Highlights:A Qualified Professional (10A NCAC 27G must have one of the following:
Bachelor degree (non Human Services) with 4 years Full time OR 8 years Part time applicable experience required
Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience required
Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience required
• Provides psychoeducation as indicated in the PCP.
• Assists the Team Leader with behavioral and substance use disorder treatment interventions.
• Assists with the development of relapse prevention and disease management strategies.
• Coordinates the initial and ongoing assessment activities.
• Develops and/ or participates in the initial development, implementation, and ongoing revision of the PCP, and ensures its implementation.
• Consults with identified medical (i.e., primary care and psychiatric) and non-medical providers, engages community and natural supports, and includes their input in the person-centered planning process.
• Ensures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
• Monitors and documents the status of the beneficiary's progress and the effectiveness of the strategies and interventions outlined in the PCP.
• Completes functional needs assessment(s) to determine the scope and anticipated outcomes to the services.
• Communicates the beneficiary's progress and the effectiveness of the strategies and interventions to the team leader as outlined in the PCP.
• Provides intensive case management.
• Identify and provide emergency crisis services as necessary and respond accordingly; coordinate other services as appropriate.
• Participate in agency on-call structure as First Responder.
• Facilitate a debriefing session following each crisis event for the purpose of making necessary changes to the PCP or crisis plan in an effort to prevent future occurrences of similar crises.
• Linkage and referral to formal and informal supports.
• Assist with beneficiary housing search including engaging landlords to rent to beneficiaries and writing reasonable accommodation letters.
• Assist with connecting beneficiaries to financial and in-kind resources to set up and maintain their household.
• Prevent and mitigate housing crises including being a point of contact for landlord concerns.
• Assist with rehousing beneficiaries if they are no longer able to stay in their unit due to eviction or risk of eviction.
• Assist in developing daily living skills to stabilize and maintain housing.
• Ensure admission and discharge procedures are followed according to policy.
• Coordinate admissions and discharges under the supervision of the team leader.
• Monitor utilization of services and ensure agency provides services consistent with authorization for each individual supported.
• Prepare reports for team leader in order to clearly communicate progress or lack of progress, and involvement in treatment to aid the team leader in the authorization process.
• Maintain timely and accurate service documentation in a manner consistent with agency and state/federal law.
• Complete and submit accurate billing information on time; maintain and ensure comprehensive medical records are kept for each person served.
• Maintain positive working relationships within the communities served, including with individuals, families, team members, other staff, monitoring and licensing agencies, organizations, and funders.
• Drive and travel extensively to community locations, various agencies, and other outreach destinations.
• Provide and/or arrange for transportation for people supported as required.
• Attend and actively participate in meetings and training as required.
• Maintain certification in all agency, state and federal training requirements.
• Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
• Demonstrate knowledge of emergency procedures and assist in crisis situations.
• Follow service definition guidelines for services being provided.
• Complete all other relevant responsibilities as assigned by the supervisor.
On-call rotation required.
