Summary
The Medical Director of the Office of Coordinated Care provides psychiatric clinical leadership and oversight for Office of Coordinated Care teams, Care Coordination teams, This position is responsible for ensuring the delivery of high-quality, person-centered, and evidence-based psychiatric care to individuals experiencing acute mental health crises, homelessness, and/or substance use disorders, often in community and street settings. This role requires a unique blend of clinical expertise, community outreach experience, and administrative skills.
Essential Duties and Responsibilities:
- Provides clinical and administrative direction and supervision to OCC and BHS Linkage Medication Support Services psychiatrists, psychiatric nurse practitioners, and other medical staff as assigned. Oversees the psychiatric assessment, diagnosis, treatment planning, and medication management of individuals served by OCC Psychiatric providers, with a focus on crisis stabilization, harm reduction, and engagement in ongoing care
- Provides direct patient care, which includes evaluating, diagnosing, and providing medication management, including medications for addiction treatment (MAT), to people at BHAC, on the street, and discharging from acute settings such as hospital and jail, in person and via telehealth.
- Assesses people for grave disability and refers for LPS conservatorship, as appropriate.
- In partnership with OCC leadership, develops, implements, and monitors clinical protocols, policies, and procedures for OCC programs, ensuring alignment with evidence-based practices, regulatory requirements, and ethical standards.
- Provides clinical direction and supervision to psychiatrists and psychiatric nurse practitioners within Street Health Psychiatry.
- In partnership with Street Health and Whole Person Integrated Care (WPIC) leadership, develops, implements, and monitors clinical protocols, policies, and procedures for Street Health Behavioral Health, ensuring alignment with evidence-based practices, regulatory requirements, and ethical standards.
- Partners with Street Health and WPIC and across SFDPH to support psychiatric care, and ensure psychiatry participation in collaboration with community partners, law enforcement, and other stakeholders to identify and engage individuals in need.
- In partnership with Street Health, WPIC, and OCC leadership, ensures efficient and effective triage, assessment, and referral services to community programs and other resources.
- Participates in quality improvement initiatives and audits, including data collection, analysis, and implementation of corrective actions to improve outcomes and service delivery
- Serves as a liaison with community providers, hospitals, shelters, law enforcement, and other relevant stakeholders to facilitate continuity of care and improve system coordination
- Represents the Psychiatric Services under OCC and Street Health in community forums, meetings, and collaborations
- Maintains current knowledge of relevant research, clinical guidelines, and local, state, and federal legal/regulatory changes related to behavioral health, homelessness, and substance use disorders
- Ensures compliance with all applicable federal, state, and local regulations, including HIPAA and confidentiality requirements
- Works collaboratively with other programs within BHS and across departments, including BEAM, Street Health, and Whole Person Integrated Care (WPIC) to ensure seamless transitions of care.
- 20-40 % time provides direct patient care in an OCC program
- 20 % time provides consultation and training to staff on psychiatric issues, including on complex psychiatric cases seeking services, crisis intervention, de-escalation techniques, harm reduction strategies, and trauma-informed care
- 40-60 % time Administrative, program planning, supervision, etc.
Qualifications
- Doctor of Medicine or Osteopathic Medicine degree (M.D. or D.O.) required
Experience
- 5 years of providing Psychiatric services (post-residency) including SUD - treatment
Certificates and Licenses
- Valid U.S. Medical or Osteopathic License
- Board eligible or board certified in Psychiatry

Pay: $304, $439,842.00 per year
