Job Summary: This position is a Clinical Pharmacist, working with physicians, nurse practitioners, and other clinical staff at Heart of Ohio Family Health as part of a medical care team focusing on patient outcomes. The Clinical Pharmacist provides patients and providers with therapeutic assessments and selected disease management, for assigned practices, working toward improving patient and practice outcomes. In addition to direct patient care in the clinic, the Clinical Pharmacist will also support the 340B program, fill in at the retail pharmacy, and participate in population health initiatives.

Dress Requirement: Business casual in accordance with Heart of Ohio Family Health dress code policy

Work Schedule: F/T

Monday through Friday during standard business hours

Facility Environment:

Heart of Ohio Family Health operates in multiple locations, in Columbus, OH area. All facilities have a medical office environment with front-desk reception area, separate patient examination rooms, nursing stations, pharmacy stock room, business offices, hallways and private toilet facilities. All facilities are ADA compliant.

This position's primary work area is the clinical pharmacist office, patient examination rooms, and nursing stations.

The patient examination rooms, and nursing stations areas are:

  • kept at a normal working temperature
  • sanitized daily
  • maintains standard office environment furniture with adjustable chairs
  • maintains standard office equipment, i.e., computer, copier, fax machine, etc. at a normal working height

Physical Demands and Requirements: these may be modified to accurately perform the essential functions of the position:

  • Mobility = ability to easily move without assistance

  • Lifting/Carry = ability to lift and carry a normal stack of documents and/or files

  • Speaking = ability to pronounce words clearly to be understood by another individual

ESSENTIAL FUNCTIONS:

  • Assists physicians and mid-level providers in creating and managing the drug regimens of patients with chronic disease states (e.g. diabetes, hypertension, dyslipidemia, asthma, COPD, HIV treatment and PrEP, Hepatitis C, etc.). This may include, but shall not be limited to, activities such as meeting with patients, adjusting medication dosages and monitoring labs in accordance with a consult agreement, providing education, and performing other services within the professional area of expertise.
  • Performs medication reconciliation assessments as requested by Heart of Ohio Family Health providers and/or case managers to optimize the patient's drug regimen.
  • Assists in the selection of the most cost effective, evidence-based regimen for a particular patient, monitors patients for appropriate lab and other recommended follow-up.
  • Monitors patient response to treatment, drug/drug and drug/disease interactions; assesses medications prescribed at dosages higher than approved by the FDA for specific indications.
  • Coordinates/participates in activities related to improving outcomes and UDS measures for targeted disease management initiatives leading to improved health (such as diabetes, hypertension, asthma, and others).
  • Identifies patient/family gaps in knowledge of diagnosis and/or treatment and development interventions with patient/family input to mitigate gaps.
  • Provides clinical pharmacy input during the preparation of interdisciplinary care plans for patients.
  • Assists providers as needed to meet Patient-Centered Medical Home Certification (PCMH).
  • Embedment in multiple practices across all sites as part of the medical care team.
  • Coordinates referrals with dietitians, community health workers, behavioral health team members, etc. as needed.
  • Makes referrals to the appropriate Medicaid experts for Medicaid contractual and coverage issues.
  • Serves as a drug information resource for the providers and support staff.
  • Provides highly specialized education and training support to other professional services in the broad area of advanced pharmacotherapy, with emphasis on chronic disease states.
  • Utilizes existing computer resources to maximize retrieval of clinical information for monitoring and review of the medication use process.
  • Serves as a resource to providers and case managers on general drug information, 340B program, and medication formulary issues.
  • Completes prior authorizations as needed, including prior authorizations for select specialty medications (e.g. Hepatitis C and HIV medications).
  • Cross-trains in retail pharmacy operations and provides select lunch and emergency coverage to ensure continuity of patient care and workflow efficiency.
