NewYork-Presbyterian has a long tradition of bringing hope, health and healing to our communities. NYP is continuously implementing new programs to meet the unique needs of our communities, and is strengthening those that exist. Our programs build upon, and tie together, the Hospital's inpatient care coordination, Ambulatory Care Network, and community outreach programs. Now, we're seeking a Program Navigator to help drive this work forward. NYPH was recently selected to be one of five "Youth Opportunity Hubs," a first-of-its-kind effort to knit together community-based providers funded through the Manhattan District Attorney Office's Criminal Justice Investment Initiative.
Youth Advocate – Uptown Hub - Columbia
Under the supervision of the Program Manager, the Youth Advocate is responsible for assisting in Hub member enrollment and engagement, program support, working one-on-one with assigned cases, and other duties as assigned. The Hub Advocate provides direct services to individual members, including vocational advisement and task-specific guidance in goal obtainment and connecting to resources and opportunities both within the hospital and the community.
Specific Job Duties
- Sustain a working understanding of all patient confidentiality requirements (HIPAA, NYP) and work with the Hub staff to implement privacy policies and best practices to protect Hub participant information.
- Identify and assess Hub member needs to determine appropriate referrals, intervention, risk triage, and/or services needed.
- Coordinate care of Hub member with other members of their care team within NYP, including weekly meetings with the Hub's Behavioral Health team to triage, discuss and plan care regarding behavioral health needs of the participant.
- Carry a caseload of up to 50 Hub members providing them with structured or unstructured supportive guidance in goal-oriented work that can include, but is not limited to, regular in-person meetings, scheduling appointments, following up on program attendance, and facilitating warm handoffs to Hub partners and other outside services.
- Make appropriate referrals to resources to facilitate educational, vocational, and behavioral interventions recommended for Hub member success.
- Administer Hub screens to members.
- Using the supportive guidance notes template, document outreach and work done with Hub members and referral connections in Epic.
- Perform concurrent review of referrals, utilizing a multi-disciplinary approach.
- Support planning and hosting of Hub programs and special events, such as member project exhibitions and performances.
- Research, maintain, organize, and keep track of resources within the hospital and throughout the community.
- Attend training conferences offered by the department as well as by outside organizations.
- Perform other job related duties as assigned.
Preferred Criteria
- Experience working with LGBT-identified, runaway and homeless, foster care and/or immigrant youth populations
- Experience with program planning, development, and evaluation
- Demonstrate an understanding of and practice the principals of positive youth development
- Fluent Spanish
Required Criteria
- Bachelor's degree or a minimum of 2 years of equivalent experience in primary or community health
- Excellent oral and written communication skills
- Knowledge of community needs and of the Hospital and community resources to fill those needs
