End Date: 13-week contract

Contract Type: Contract

Shift Type: AM Shift (Mon–Fri, 8 AM–5 PM; flexible hours)

Hours per Week: 40 hrs

Are you a skilled and compassionate Physical Therapist looking to make a meaningful impact in post-acute care? Join our home health team in Santa Maria, CA, where you'll provide personalized therapy services that help patients regain independence and mobility in the comfort of their homes.

Key Responsibilities

  • Deliver home-based physical therapy services to patients across assigned geographical regions
  • Conduct evaluations and develop individualized treatment plans using Oasis documentation
  • Treat patients with orthopedic, neurological, and post-surgical conditions including strokes, joint replacements, and spinal injuries
  • Collaborate with OT, Speech, and Rehab Aides/PTAs to ensure coordinated care
  • Maintain accurate records using Cerner EMR
  • Participate in interdisciplinary meetings and provide education to patients and caregivers

Qualifications

  • Minimum 2 years of PT experience
  • Experience with Oasis documentation required
  • Home health experience preferred but not mandatory
  • BLS Certification required
  • Licensed Physical Therapist in California

Education

  • Graduate of an accredited Physical Therapy program

Knowledge & Skills

  • Strong clinical reasoning and patient assessment skills
  • Ability to work independently in a home setting
  • Excellent communication and documentation abilities
  • Familiarity with adaptive equipment and therapeutic techniques
  • Comfortable with Cerner EMR system
