FLSA Status: Full-Time, Non-Exempt
Hours: 35 hours per week
Schedule: Includes one (1) evening shift per week and two (2) Saturdays per month
Location: Hartford, CT (community-based position with regional travel)
Hourly Rate: $23.00 – $26.00 per hour (commensurate with experience)
Position Summary
The Community Case Manager – High-Risk Populations plays an essential role in advancing CTHRA's mission to promote health equity, reduce harm, and improve the quality of life for individuals disproportionately impacted by substance use, housing instability, sex work, and systemic inequities. This position provides community-based outreach, case management, and linkage-to-care services to individuals who face barriers accessing traditional systems of support.
The ideal candidate demonstrates empathy, strong cultural humility, and a trauma-informed, nonjudgmental approach. This role requires flexibility, consistency, and the ability to engage individuals with complex needs through trust, respect, and advocacy.
Key Responsibilities
Conduct intakes and individualized needs assessments to identify participant strengths, goals, and barriers. Develop and implement service plans focused on housing stability, harm reduction, health access, and community reintegration. Provide consistent case management, coordination, and referrals to healthcare, behavioral health, social service, and housing providers. Offer harm reduction supplies (naloxone, safer use kits, condoms, fentanyl test strips) and provide education on safer drug use and overdose prevention. Support participants in navigating systems (e.g., DSS, healthcare, criminal justice) while empowering self-advocacy and autonomy. Conduct regular street-based and community outreach to engage individuals who use drugs, are unstably housed, or engage in survival-based economies. Build rapport through trauma-informed, culturally responsive communication. Represent CTHRA in the community by promoting harm reduction services, mobile outreach, and participant-centered support. Collaborate with outreach teams to identify emerging community needs and adapt service delivery accordingly. Work one (1) evening shift each week to meet participants outside traditional business hours and two (2) Saturdays per month as part of CTHRA's rotating outreach and engagement schedule. Maintain flexibility to respond to community events, emergency outreach, or seasonal needs. Maintain accurate and timely documentation of all participant interactions, referrals, and outcomes using CTHRA's data management systems. Track metrics and deliverables for program evaluation and grant compliance. Adhere to confidentiality and privacy standards under HIPAA, 42 CFR Part 2, and agency policy. Work closely with Program Managers, Outreach Specialists, Peer Navigators, and partner agencies to ensure seamless service coordination. Participate in multidisciplinary case conferences, supervision, and staff meetings. Advocate for participants' rights and equitable access to care across public and private systems. Serve as a bridge between participants and community resources, helping reduce stigma and systemic barriers. Participate in agency-required training on harm reduction, trauma-informed care, motivational interviewing, and racial equity. Engage in continuous learning to remain current on best practices in public health and social services. Contribute to team culture by modeling compassion, accountability, and professional integrity.
Qualifications
Required:
- Bachelor's degree in Social Work, Human Services, Public Health, or related field or equivalent lived/professional experience.
- Minimum of two (2) years of experience providing outreach, case management, or direct services to individuals who use drugs, are unhoused, or face systemic barriers.
- Demonstrated understanding of harm reduction philosophy, public health principles, and trauma-informed engagement.
- Strong organizational, communication, and boundary-setting skills.
- Valid Connecticut driver's license and reliable transportation.
- Ability to work evenings and Saturdays as required.
Preferred:
- Lived experience with substance use, recovery, homelessness, or sex work.
- Familiarity with local service systems (housing, healthcare, behavioral health, criminal justice).
- Proficiency in using databases or case management software.
Working Conditions
- Work is performed in both office and community environments, including street outreach and community events.
- Regular exposure to outdoor elements, varying weather, and community-based settings.
- Must be comfortable engaging individuals in active substance use or experiencing housing instability.
- Use of agency vehicle or personal vehicle (mileage reimbursed) required for outreach.
