Job Summary

We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide comprehensive therapy services to diverse patient populations, including pediatrics, geriatrics, and individuals with disabilities. This role involves developing and implementing personalized care plans to enhance patients' functional abilities, independence, and quality of life across various settings such as inpatient facilities, outpatient clinics, home health, schools, and skilled nursing facilities.

Duties

  • Assess patients' physical, cognitive, and emotional needs through detailed evaluations.
  • Develop and implement individualized treatment plans focused on improving activities of daily living (ADLs), mobility, and functional independence.
  • Assist patients with ADLs and IADLs to promote independence in daily routines.
  • Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient transitions across care settings.
  • Work with diverse populations including children with autism or developmental delays, adults in post-acute care, and seniors in geriatrics or skilled nursing facilities.
  • Provide therapy interventions for patients in acute care, ICU, inpatient, outpatient, home health, or school environments.
  • Maintain accurate documentation of patient progress and adjust treatment plans accordingly.
  • Engage in case management activities to support long-term patient outcomes.
  • Educate patients and families about therapy goals, strategies, and home programs to support ongoing recovery.
  • Stay current with clinical best practices related to physiology, anatomy, medical terminology, and speech therapy as applicable.

Requirements

  • Valid state licensure as an Occupational Therapist.
  • Proven experience working with people with autism, disabilities, or in pediatric and geriatric populations.
  • Experience in acute care settings such as ICU or inpatient units preferred.
  • Knowledge of care plans, case management principles, and discharge planning processes.
  • Familiarity with medical terminology, physiology, anatomy knowledge necessary for effective therapy delivery.
  • Experience working in various environments including skilled nursing facilities, outpatient clinics, home health services, schools, or post-acute care settings.
  • Strong patient care skills with the ability to work compassionately with diverse populations including pediatrics and geriatrics.
  • Excellent communication skills for collaborating with healthcare teams and educating patients/families.
