The desired candidate must be licensed in the State of Georgia as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or a Licensed Professional Counselor (LPC), Master Social Worker (MSW), Associate Marriage and Family Therapist (AMFT), or Associate Professional Counselor (APC) to apply. The mental health professional will be conducting assessments to assess the severity of the presenting symptoms and recommend care as necessary.

We provide services in Macon, Warner Robins, Fort Valley, Byron, Centerville, Perry, Forsyth, and a few others.

Job Description:

  • Assessor will thoroughly review and assist consumer in completing the intake packet for services making sure that the consumer has a complete understanding of the forms signed and overview of the program.

  • Assessor will collect information from the consumer, natural supports, and/or community agencies regarding mental health, substance use, behavioral issues, medical health, education, the effects of symptoms on activities daily living, and family history of mental and medical health.

  • Assessor will develop treatment goals and objectives to assist the consumer in meeting their stated goals based on the information provided by the individual and/or parent/guardian.

  • Assessor must prepare and submit clinical documentation by the designated deadlines to ensure compliance.

  • Assessor will work independently and collaboratively with other staff to ensure cases are properly managed according to the need of clients.

  • Ensure confidentiality and rights of individuals served are maintained at all times.

  • Attend necessary meetings as required.

Qualifications & Skills

  • Full and Associate Licensure (LCSW, LMFT, LPC, MSW, AMFT, APC ) in the State of Georgia

  • Experience in community-based or home-based assessment and/or counseling experience

  • Training and/or experience in industry accepted clinical assessment tools

  • Must possess a valid Georgia driver's license and have reliable transportation.

  • All Applicants must be willing to submit to a background check and drug screen if selected.

Pay: From $38.00 per hour
