Job Summary: We are seeking a skilled and detail-oriented MDS Coordinator to join our team. The MDS Coordinator will be responsible for managing and coordinating the Minimum Data Set (MDS) process for our facility. This includes ensuring accurate and timely completion of assessments, maintaining compliance with regulations, and collaborating with healthcare professionals to develop individualized care plans. The ideal candidate will have a strong background in health information management and a passion for providing quality patient care. Responsibilities: - Coordinate the MDS process, including assessment, documentation, and submission - Ensure accuracy and completeness of all MDS assessments - Collaborate with interdisciplinary team members to develop individualized care plans - Monitor and track resident progress and update care plans as needed - Maintain compliance with HIPAA regulations and other applicable guidelines - Stay up-to-date with changes in coding systems (ICD-9, ICD-10) and reimbursement methodologies (DRG) - Conduct audits to ensure data integrity and quality improvement - Provide education and training to staff on MDS documentation requirements Skills: - Strong knowledge of health information management principles and practices - Proficiency in patient care coordination in an acute care or home care setting - Familiarity with coding systems such as ICD-9 and ICD-10 - Understanding of reimbursement methodologies, including DRG - Excellent attention to detail and organizational skills - Ability to work collaboratively with interdisciplinary teams - Knowledge of HIPAA regulations and compliance requirements - Strong communication skills, both written and verbal - Proficiency in using computer systems and electronic medical records

Job Type: Full-time

Pay: From $35.00 per hour
