Position Summary

The Case Manager provides comprehensive case management services to patients through psychosocial assessment, intervention, and follow-up. The Case Manager is a member ofthe Behavioral Health Department and works closely with team members to ensure that appropriate care and services are provided to patients.

  • Comply with Camillus Health Concern's Mission, Vision, and Values
  • Conduct full assessment of each patient's medical, financial, social and other needs.
  • Provide patients with supportive short-term and crisis counseling.
  • Provide information and education on the Social Security Disability and Medicaid processes and other government programs.
  • Provide comprehensive case management services to clients through intervention, referrals and follow-up including patients who have been hospitalized.
  • Provide assistance to PCP to request Medical Equipment (supplements, wheelchairs, hearing aids, eyeglasses, insoles) through insurance as needed.
  • Provide care planning through identifying, implementing and management of community service programs for client including Camillus House services.
  • Provide bus passes to eligible clients.
  • Monitor client compliance with care plan and medication regimens, as well as ensuring that service providers are rendering services as requested
  • Follow-up with clients who have relocated to less supportive housing situations
  • Document all patient encounters in the health record.
  • Provide Photo Identification cards
  • Submit monthly report on case management
  • Work effectively and professionally with Camillus staff and community partners.
  • Participate in case conferences and supervisory sessions
  • Perform any other department or agency related duties or special projects as directed by Supervisor.

Reuirements

  • Bachelor's degree in social work or related field; previous experience and certification(s) considered.
  • Knowledge of and experience with public health issues.
  • Bilingual (English/Spanish) preferred, Creole a plus.
  • Ability to work in a highly diversified environment.
  • Maintains active valid licensure and credentials.
  • Excellent organizational and documentation skills
