Role Description

This is a full-time, on-site Social Worker role at our Brooklyn, NY facility. The Social Worker will provide emotional support and resources for patients and their families, ensuring they navigate healthcare and community systems effectively. Responsibilities include conducting psychosocial assessments, developing individualized care plans, coordinating discharge processes, and collaborating with interdisciplinary teams to provide comprehensive care. The Social Worker will also advocate for patients and connect them with relevant community resources and support networks.

Qualifications

  • Strong counseling, communication, and interpersonal skills
  • Knowledge of case management, psychosocial assessments, and care planning
  • Ability to coordinate resources and provide referrals to community support services
  • Experience in crisis intervention, advocacy, and problem solving
  • Familiarity with healthcare systems and regulatory requirements
  • Capacity to work collaboratively with interdisciplinary teams
  • Bachelor's or Master's degree in Social Work or a related field is required
  • Relevant certifications or licensure such as LMSW or LCSW are preferred
  • Prior experience in a healthcare or long-term care setting is a plus
