The Flex Licensed Practical Nurse (LPN) is responsible for providing direct care to the residents and supervision over the day to day activities of the nursing staff and nursing aides in accordance with current federal and state standards, guidelines, and regulations that govern the Smith Care Center including Plymouth Harbor policies and procedures.

DUTIES/RESPONSIBILITIES

  1. Assist in the planning, development, organization, implementation, evaluation, and direction of assigned shifts' activities.

  2. Meet with administration, medical and nursing staff, and related departments in planning department services, programs, and activities.

  3. Assist in development, implementation, and maintenance of nursing objectives and standards.

  4. Assist in development, implementation, and coordination of departmental policies and procedures, resident care plans, nursing procedure manuals, and job descriptions.

  5. Ensure that departmental personnel, residents, and visitors follow established policies and procedures at all times.

  6. Be involved in the planning and implementation of recommendations by the Care Plan Team, RISK Committee, Utilization Review Committee, and Medicare Stand Up meetings.

  7. Ensure that documentation of residents' daily medical record is accurate, comprehensive and timely.

  8. Receive, document, and implement physicians' orders in an accurate and timely manner.

  9. Coordinate admit and discharge of residents to and from the Smith Care Center.

  10. Ensure timely response to resident requests/call lights.

  11. Coordinate Home Care services and Smith Care Center services as indicated.

  12. Ensure the proper recording and accurate count of schedule drugs at all times.

  13. Ensure security of locked Smith Care Center areas; monitor that only authorized staff is in possession of Smith Care Center keys.

  14. Ensure accurate and comprehensive reporting of resident condition via shift to shift reporting.

  15. Interpret departmental policies and procedures to personnel, residents, visitors, and family members.

  16. Visit each resident at least once during a shift to ensure appropriate care is being rendered and act accordingly.

  17. Ensure that Residents' Bill of Rights is followed at all times.

  18. Ensure that residents have a clean, safe, orderly, and comfortable environment.

  19. Assist in the development of and participate in programs designed for in-service education. Monitor orientation for unlicensed new hires. Orient licensed new hires.

  20. Establish and maintain rapport with the medical profession and other health related facilities and organizations.

  21. Assist in identifying departmental problems and subsequent resolutions.

  22. Make recommendations to the Director of Health Services regarding staffing, improvement of services, and potential problem areas.

  23. Participate in discharge planning, development and implementation of resident care plans and resident assessments.

  24. Perform administrative duties as required by departmental policies and procedures and report to the Director of Health Services.

  25. Assist in coordinating transportation to other health care facilities and individuals as indicated.

  26. Participate in departmental projects and studies as assigned.

  27. Coordinate health services, i.e., admissions, discharges, etc.

  28. Ensure that a stock level of medications, medical supplies, equipment, and other supplies is maintained.

  29. Inspect nursing care service areas and practices regularly to ensure compliance with current applicable regulations.

  30. Cooperate with resident's physician in planning nursing care; periodically reviewing and updating care plans with MDS Coordinator.

  31. Complete medical forms according to departmental policies and procedures.

  32. Notify the resident's attending/alternate physician, family, or guardian, of any change in condition, accident or incident according to established facility policy.

  33. In cases of death, implement arrangements according to resident's, family's, or guardian's wishes and in compliance with facility policy.

  34. Evaluate emergencies, determine emergency measures to be used, and notify the Director of Health Services of the emergency.

  35. Assist in the standardization of methods in which work will be accomplished.

  36. Collect laboratory specimens as ordered.

  37. Assist in establishing the administration, procedures, and control of medications, etc., as recommended by the pharmacists/Pharmacy Committee.

  38. Perform prescribed monitoring and diagnostic and therapeutic procedures.

  39. Assist medical professionals, as needed, during on-site visits.

  40. Follow Universal Precautions and Infection Control procedures. Adhere to isolation techniques when appropriate.

  41. Comply with Florida State statutes regarding required continuing education credits.

  42. Attend and participate in workshops, seminars, and mandatory in-services as approved or required.

  43. Maintain work area, equipment and supplies in a clean, sanitary and organized manner. Ensure that work areas are neat and confidential materials are properly stored before leaving area on breaks, end of work day, etc.

  44. Work safely; complying at all times with Plymouth Harbor safety standards.

  45. Call 555 in the event of an emergency.

  46. Discharge properly such other responsibilities and duties as the Director of Health Services may direct.

SUPERVISORY RESPONSIBILITY

  • The Flex LPN has supervisory responsibility for Certified Nursing Assistants and Housekeepers.

QUALIFICATIONS

Education:

  • Graduate of an accredited school of licensed practical nursing.

Experience:

  • Minimum one year in the field of Long-Term Care preferred.

General:

  • Must have good organizational skills.
  • Must be a detail-oriented person and able to perform duties with great accuracy on a daily basis.
  • Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual's competence.
  • Must have good communication skills; speaking, writing, and listening.
  • Must be fluent in English.
  • Must have good computer skills with the ability to learn and use proprietary software as required.

CERTIFICATES/LICENSES/REGISTRATIONS

  • Must have valid certification as a Licensed Practical Nurse in the State of Florida.
  • Must have a current CPR (Cardiopulmonary Resuscitation) certification.
