Duties
- Provide compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Monitor and document patient observations such as vital signs, medication adherence, and overall health status using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Assist with medical procedures such as catheter care, medication administration, and wound care under the supervision of healthcare professionals.
- Support clients with mobility using equipment like Hoyer lifts and ensure safety during transfers and ambulation.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment to promote hygiene and comfort.
- Maintain accurate medical documentation in compliance with HIPAA regulations to ensure confidentiality and proper recordkeeping.
- Support clients with specialized care needs such as dementia, Alzheimer's disease, memory care, hospice services, or working with individuals with developmental disabilities or disabilities.
- Engage clients in meaningful activities to promote mental stimulation and emotional well-being.
- Collaborate effectively with family members, caregivers, nurses, and other healthcare professionals to deliver comprehensive home health services.
- Ensure compliance with all safety protocols and participate in ongoing training related to CPR certification, medical terminology updates, and best caregiving practices.
Qualifications
- Proven experience as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home & community care environments.
- Strong knowledge of patient observation techniques, vital signs measurement, medication administration, and dementia or Alzheimer's care.
- Familiarity with EMR/EHR systems for medical documentation and recordkeeping.
- Ability to assist with ADLs effectively while maintaining dignity and respect for clients' independence.
- Experience working with people with disabilities or developmental challenges is highly desirable.
- Certification in CPR is required; additional certifications such as hospice care training or catheter care are a plus.
- Knowledge of medical terminology relevant to home health services.
- Ability to work independently while collaborating within a multidisciplinary team.
- Excellent communication skills coupled with patience, empathy, and a positive attitude toward caregiving roles.
- Valid driver's license preferred for travel between client locations; reliable transportation is essential.
