What You'll Do:

  • Leverage your existing network across hospital discharge planners, SNFs, assisted living facilities, physician offices, and care managers.
  • Build and grow new relationships with hospital discharge planners, SNF case managers, physicians and other healthcare providers.
  • Educate healthcare providers on the agency's services and the value we bring to their patients.
  • Own the growth of our referral base—with full internal support from our operations and clinical teams.
  • Represent our agency with the confidence and credibility of someone who knows the home health game inside and out.
  • Work independently while being backed by a responsive and agile leadership team that knows how to get things done.
  • Develop the company's marketing strategies, campaigns and programs that align with market analysis and company targets.
