St. Patrick's Center strives to serve our community with dignity, compassion, and kindness. The goal of the Case Manager is to provide clients with wrap around services to help overcome barriers to stability. The Case Manager is responsible for carrying a monthly caseload of 35 individuals per month and seeing individuals for one-time referrals, estimated at an additional 40 individuals/month. For case managed individuals, services provided will focus on the core areas of housing, employment, and social/physical wellness.

This role requires inputting client data into Captains case management software to establish intake and track growth and achievement of goals of each client. Additionally, monthly reporting to multiple stakeholders and grant and contract reporting and management is required.

Job Duties

Program Coordination

  • Ensure timely and accurate monthly reporting to DSAMH, Fist State CAA, and other stakeholders.
  • Provide customized intake for clients to assess their level of need and support services
  • Maintain monthly caseload of 35 individuals, developing goals and identifying objectives to reach goals
  • Conduct a minimum of one monthly visit or follow up per month with all case managed clients (located onsite at St. Patrick's Center- no travel required)
  • Track client progress in Captains database system, monitoring each individual's growth and achievement of goals
  • Connect clients to external organizations/ resources as needed
  • Monitor challenge areas and propose changes to Program Director
  • Maintain updated information and resource packets for core areas: Housing, employment, social/ physical health
  • Respond to all questions, concerns, requests in a professional and non-confrontational manner
  • Provide exemplary customer service: Treating all clients with dignity and respect, de-escalating difficult situations while maintaining boundaries
  • Attend workshops, trainings, staff meetings as required

Skills and Qualifications

  • Bachelor's degree or Master's degree in social work or human services, minimum of 2 years' experience working with the homeless or other vulnerable populations
  • Licensed social worker strongly preferred
  • Exemplary customer service, working well under pressure
  • Ability to conduct monthly reporting, maintain caseload files electronically
  • Demonstrated experience working cross culturally and ability to build trust and build rapport with individuals of all backgrounds
  • Experience working with social support systems locally preferred
  • Good organizational and time management skills
  • Attention to detail, experience reporting to multiple government stakeholders preferred
  • Ability to lift up-to 25 pounds. This position requires walking, standing, sitting, and the ability to work a flexible schedule, including occasional evenings and weekends.
