Case Manager – Serenity Human Service Coordination

Full-Time | Preferred Bilingual (English/Spanish)

Location: Idaho Falls

Schedule: Monday–Friday 8 am - 6 pm with an hour lunch

Salary: Starting pay is $22 per hour, plus an additional $2 for every billable unit completed. The additional $2 is paid as untaxed reimbursement since case managers are considered community workers.

Serenity Human Service Coordination is a growing behavioral health agency committed to providing compassionate, high-quality support to children, adults, and families in our community. We are looking for a dedicated and organized Case Manager to join our team and support clients in navigating services, resources, and individualized goals.

Position Summary

The Case Manager will work directly with clients, families, and interdisciplinary teams to coordinate services, maintain client records, and ensure quality care. The ideal candidate is bilingual (English/Spanish), highly organized, empathetic, and able to work both independently and collaboratively.

Responsibilities

  • Provide case management services to assigned clients based on needs and treatment plans.
  • Complete intakes, assessments, and service plans.
  • Maintain accurate and timely documentation, including progress notes, authorizations, and trackers.
  • Coordinate with counselors, administration, schools, doctors, and community partners.
  • Advocate for client needs and ensure continuity of care.
  • Attend staff meetings, trainings, and supervision.
  • Maintain confidentiality and comply with agency/state regulations.

Requirements

  • Bachelor's degree in Human Services, Social Work, Psychology, or related field (required).
  • Bilingual English/Spanish strongly preferred.
  • Strong organizational and communication skills.
  • Ability to work independently with time-management skills.
  • Professional, compassionate, and able to work with diverse populations.
  • Must pass a background check and complete required trainings.
