Occupational Therapist (OT) � Skilled Nursing & Rehabilitation | Kingston Care Center of Fort Wayne, IN

Full-Time or Part-Time | On-Site | Indiana OT License Required

Kingston Care Center of Fort Wayne is hiring a compassionate and motivated Occupational Therapist (OT) to join our skilled nursing and rehabilitation team. In this role, you�ll help residents regain independence, improve daily living skills, and achieve greater quality of life�all within a supportive, mission-driven care environment in Fort Wayne, Indiana.

What You�ll Do as an Occupational Therapist

  • Evaluate residents� abilities and develop individualized therapy treatment plans

  • Provide therapy to improve self-care, fine motor skills, mobility, and independence in daily activities

  • Track progress, adjust therapy plans, and maintain accurate records

  • Collaborate with physicians, nurses, physical therapists, and interdisciplinary team members

  • Educate residents and families on adaptive strategies, therapy goals, and safe practices

  • Ensure therapy services comply with Indiana state regulations and Kingston�s standards of care

What You�ll Need

  • Active Indiana Occupational Therapist (OT) license required

  • Bachelor�s, Master�s, or Doctorate degree in Occupational Therapy from an accredited program

  • Experience in skilled nursing, rehabilitation, or long-term care preferred (new grads welcome)

  • Strong clinical judgment, organizational, and communication skills

  • Compassionate, resident-first approach to therapy

  • Ability to thrive independently and as part of a collaborative team
