Job Summary
We are seeking a dedicated and dynamic Registered Nurse (RN) Case Manager to join our healthcare team. Your expertise will drive effective discharge planning, utilization management, and post-acute care coordination, empowering patients on their journey to recovery and wellness. Duties
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.
- Develop individualized care plans in collaboration with physicians, specialists, and other healthcare providers to promote efficient recovery pathways.
- Coordinate inpatient and outpatient services, ensuring smooth transitions from hospital to post-acute or community settings.
- Manage utilization review processes by evaluating diagnostic evaluations, medical records, and treatment plans to optimize resource use while maintaining quality care.
- Lead discharge planning efforts by assessing patient needs, arranging appropriate post-acute services, and educating patients and families about ongoing care requirements.
- Monitor patient progress through ongoing assessment and adjust care plans accordingly to meet evolving health goals.
- Maintain accurate documentation in medical records while adhering to HIPAA regulations and ensuring confidentiality of sensitive information.
- Collaborate with multidisciplinary teams in emergency room, ICU, PICU (Pediatric Intensive Care Unit), Geriatrics, Behavioral Health, and other specialized units to support complex cases requiring specialized knowledge such as trauma center protocols or infectious disease care.
- Participate in utilization management activities within managed care frameworks to ensure appropriate service delivery while controlling costs.
- Facilitate diagnostic evaluations and coordinate outpatient clinic visits for comprehensive patient support across various healthcare settings.
Requirements
- Valid RN license with current registration in the state of practice.
- Critical care experience preferred, including ICU or emergency medicine backgrounds.
- Experience working in Level I or Level II trauma centers is highly desirable.
- Strong knowledge of hospital medicine, medical-surgical nursing, and acute care procedures.
- Familiarity with infectious disease management and post-acute care coordination.
- Proven ability in patient assessment, discharge planning, and utilization management processes.
- Excellent understanding of medical terminology, physiology principles, and diagnostic evaluation techniques.
- Experience with inpatient and outpatient settings including clinics or managed care environments.
- Strong communication skills for effective patient service delivery and interdisciplinary collaboration.
