Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine individual needs.
- Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and safety protocols.
- Assist with medical procedures such as IV infusions, catheterizations, wound suturing, and sterile dressing changes using aseptic techniques.
- Document all patient interactions meticulously using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems like Epic or Cerner, ensuring compliance with medical documentation standards.
- Provide patient education on medication management, disease processes such as dementia or Alzheimer's care, and health coaching tailored to specific conditions like pain management or infectious diseases.
- Support triage activities by evaluating patient symptoms and determining urgency levels for emergency room or urgent care settings.
- Collaborate with interdisciplinary teams including physicians, case managers, and therapists to coordinate discharge planning and post-acute care services.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
- Proven experience in various clinical environments such as hospital medicine, long-term care facilities, hospice care, or emergency medicine.
- Demonstrated proficiency with accurate medical documentation.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and CPT coding for procedures.
- Experience administering injections, performing venipuncture or phlebotomy, and managing IV infusions in diverse patient populations including pediatrics and geriatrics.
- Ability to work independently on a PRN schedule while maintaining excellent patient care standards in fast-paced settings such as ICU or Level I trauma centers.
- Excellent communication skills with the ability to assess patients effectively and collaborate within multidisciplinary teams.
- Knowledge of infection control protocols and HIPAA compliance to ensure patient safety and confidentiality.
